Resources for Advanced Neuro & Spinal Surgery Patients

At Advanced Neuro & Spinal Surgery, we understand that insurance benefits and requirements can be confusing and complicated. We have compiled a list to help you understand the financial implications that might result from receiving medical care.

If you need further explanation of your benefits, please contact our office and schedule an appointment with our Insurance and Billing Department. We are here to assist you in your healing process.

Insurance Plans:

Advanced Neuro & Spinal Surgery accepts most major insurance plans, including Medicare and Medicaid (Indiana and Illinois). However, it is the patient's responsibility to ensure that we are a participating provider in your health insurance plan.

Major Insurance Plans Accepted:

Insurance Card

It is important to bring both your insurance card and a photo idea to every appointment. Please advise our staff if your insurance information has been changed and if your personal information (address, phone, etc.) has recently changed. It is important that we have this information for insurance verification and pre-certification.

Referrals

Most HMO, PPO, and POS insurance plans require a referral from your primary care provider (PCP) to a specialist. Please have your PCP contact our office to ensure a proper referral is completed prior to scheduling an appointment.

Pre-Authorization/Certification

Before any diagnostic test and/or surgical procedure, our office staff will obtain prior-authorization if required. However, it is the patient's responsibility to know his/her insurance requirements. Please note that obtaining prior-authorization does not guarantee coverage of services; your insurance must be active at the time services are rendered.

Forms of Payment

Advanced Neuro & Spinal Surgery accepts all major credit cards. To make arrangements or a payment, please contact our Insurance and Billing Department.

Workers' Compensation

We accept all workers' compensation insurance. You will be asked to provide the following information prior to your appointment:

Claim Forms

Our office staff will be happy to complete your disability, family medical leave, or loss-of-time claim forms. We ask that you please allow 10-14 days from receiving, for these forms to be completed. Please note that there is a $15 processing charge to complete each form.
